The highly successful textbook Methods for the Economic Evaluation of Health Care is now available in its third edition. Over the years it has become the standard textbook in the field world-wide. It mirrors the huge expansion of the field of economic evaluation in health care.

This new edition builds on the strengths of previous editions being clearly written in a style accessible to a wide readership. Key methodological principles are outlined using a critical appraisal checklist that can be applied to any published study. The methodological features of the basic forms of analysis are then explained in more detail with special emphasis of the latest views on productivity costs, the characterization of uncertainty and the concept of net benefit. The book has been greatly revised and expanded especially concerning analyzing patient-level data and decision-analytic modeling. There is discussion of new methodological approaches, including cost effectiveness acceptability curves, net benefit regression, probalistic sensitivity analysis and value of information analysis. There is an expanded chapter on the use of economic evaluation, including discussion of the use of cost-effectiveness thresholds, equity considerations and the transferability of economic data.

This new edition is required for anyone commissioning, undertaking or using economic evaluations in health care, and will be popular with health service professionals, health economists, pharmacists and health care decision makers. It is especially relevant for those taking pharmacoeconomics courses..

Safe Food: Bacteria, Biotechnology, and Bioterrorism (California Studies in Food and Culture, 5) Description

.

Food safety is a matter of intense public concern, and for good reason. Millions of annual cases of food "poisonings" raise alarm not only about the food served in restaurants and fast-food outlets but also about foods bought in supermarkets. The introduction of genetically modified foods—immediately dubbed "Frankenfoods"—only adds to the general sense of unease. Finally, the events of September 11, 2001, heightened fears by exposing the vulnerability of food and water supplies to attacks by bioterrorists. How concerned should we be about such problems? Who is responsible for preventing them? Who benefits from ignoring them? Who decides?

Marion Nestle, author of the critically acclaimed Food Politics, argues that ensuring safe food involves more than washing hands or cooking food to higher temperatures. It involves politics. When it comes to food safety, billions of dollars are at stake, and industry, government, and consumers collide over issues of values, economics, and political power—and not always in the public interest. Although the debates may appear to be about science, Nestle maintains that they really are about control: Who decides when a food is safe?

She demonstrates how powerful food industries oppose safety regulations, deny accountability, and blame consumers when something goes wrong, and how century-old laws for ensuring food safety no longer protect our food supply. Accessible, informed, and even-handed, Safe Food is for anyone who cares how food is produced and wants to know more about the real issues underlying today's headlines.

When Abortion Was a Crime: Women, Medicine, and Law in the United States, 1867-1973 Info

.

As we approach the 30th anniversary of Roe v. Wade, it's crucial to look back to the time when abortion was illegal. Leslie Reagan traces the practice and policing of abortion, which although illegal was nonetheless widely available, but always with threats for both doctor and patient. In a time when many young women don't even know that there was a period when abortion was a crime, this work offers chilling and vital lessons of importance to everyone.

The linking of the words "abortion" and "crime" emphasizes the difficult and painful history that is the focus of Leslie J. Reagan's important book. Her study is the first to examine the entire period during which abortion was illegal in the United States, beginning in the mid-nineteenth century and ending with Roe v. Wade in 1973. Although illegal, millions of abortions were provided during these years to women of every class, race, and marital status. The experiences and perspectives of these women, as well as their physicians and midwives, are movingly portrayed here.

Reagan traces the practice and policing of abortion. While abortions have been typically portrayed as grim "back alley" operations, she finds that abortion providers often practiced openly and safely. Moreover, numerous physicians performed abortions, despite prohibitions by the state and the American Medical Association. Women often found cooperative practioners, but prosecution, public humiliation, loss of privacy, and inferior medical care were a constant threat.

Reagan's analysis of previously untapped sources, including inquest records and trial transcripts, shows the fragility of patient rights and raises provocative questions about the relationship between medicine and law. With the right to abortion again under attack in the United States, this book offers vital lessons for every American concerned with health care, civil liberties, and personal and sexual freedom.

Experiencing Politics: A Legislator's Stories of Government and Health Care (California/Milbank Series on Health and the Public) Info

    .

John E. McDonough affords a rare glimpse into the practice of state politics in this insider's account of the fascinating interface between political science and real-life politics. A member of the Massachusetts House of Representatives for thirteen years and a skilled storyteller, McDonough eloquently weaves together stories of politics and policy with engaging theoretical models in a way that illuminates both the theory and the practice. By providing a link between scholarship and the world of experience, he communicates much about the essence of representative democracy. In the process, he demonstrates how politics extend beyond the public sphere into many aspects of life involving diverse values and interests.

McDonough describes the nature of conflict, the role of interests, agenda setting, the nature and pace of change, the use of language, and more. Accessible, insightful, and original, his stories touch on a broad range of issues—including health care politics, campaigns, and elections; a street gang called the X-men; the death penalty; campaign finance reform, and tenants versus landlords. To the author, politics is everywhere and political dynamics are universal. While the setting for this book is one legislature, the lessons and insights are intended for everyone.

Poor People's Medicine: Medicaid and American Charity Care since 1965 Info

    .

Poor People’s Medicine is a detailed history of Medicaid since its beginning in 1965. Federally aided and state-operated, Medicaid is the single most important source of medical care for the poorest citizens of the United States. From acute hospitalization to long-term nursing-home care, the nation’s Medicaid programs pay virtually the entire cost of physician treatment, medical equipment, and prescription pharmaceuticals for the millions of Americans who fall within government-mandated eligibility guidelines. The product of four decades of contention over the role of government in the provision of health care, some of today’s Medicaid programs are equal to private health plans in offering coordinated, high-quality medical care, while others offer little more than bare-bones coverage to their impoverished beneficiaries.

Starting with a brief overview of the history of charity medical care, Jonathan Engel presents the debates surrounding Medicaid’s creation and the compromises struck to allow federal funding of the nascent programs. He traces the development of Medicaid through the decades, as various states attempted to both enlarge the programs and more finely tailor them to their intended targets. At the same time, he describes how these new programs affected existing institutions and initiatives such as public hospitals, community clinics, and private pro bono clinical efforts. Along the way, Engel recounts the many political battles waged over Medicaid, particularly in relation to larger discussions about comprehensive health care and social welfare reform. Poor People’s Medicine is an invaluable resource for understanding the evolution and present state of programs to deliver health care to America’s poor.

And the Band Played On: Politics, People, and the AIDS Epidemic Info

    .

Upon it's first publication twenty years ago, And The Band Played on was quickly recognized as a masterpiece of investigatve reporting. An international bestseller, a nominee for the National Book Critics Circle Award, and made into a critically acclaimed movie, Shilts' expose revealed why AIDS was allowed to spread unchecked during the early 80's while the most trusted institutions ignored or denied the threat.  One of the few true modern classics, it changed and framed how AIDS was discussed in the following years. Now republished in a special 20th Anniversary edition, And the Band Played On remains one of the essential books of our time.
In the first major book on AIDS, San Francisco Chronicle reporter Randy Shilts examines the making of an epidemic. Shilts researched and reported the book exhaustively, chronicling almost day-by-day the first five years of AIDS. His work is critical of the medical and scientific communities' initial response and particularly harsh on the Reagan Administration, who he claims cut funding, ignored calls for action and deliberately misled Congress. Shilts doesn't stop there, wondering why more people in the gay community, the mass media and the country at large didn't stand up in anger more quickly. The AIDS pandemic is one of the most striking developments of the late 20th century and this is the definitive story of its beginnings..

Altered Genes, Twisted Truth: How the Venture to Genetically Engineer Our Food Has Subverted Science, Corrupted Government, and Systematically Deceived the Public Description

    .

This audiobook uncovers the biggest scientific fraud of our age. It tells the fascinating and frequently astounding story of how the massive enterprise to restructure the genetic core of the world’s food supply came into being, how it advanced by consistently violating the protocols of science, and how for more than three decades, hundreds of eminent biologists and esteemed institutions have systematically contorted the truth in order to conceal the unique risks of its products―and get them onto our dinner plates.

Altered Genes, Twisted Truth provides a graphic account of how this elaborate fraud was crafted and how it not only deceived the general public, but Bill Clinton, Bill Gates, Barack Obama and a host of other astute and influential individuals as well. The audiobook also exposes how the U.S. Food and Drug Administration (FDA) was induced to become a key accomplice―and how it has broken the law and repeatedly lied in order to usher genetically engineered foods onto the market without the safety testing that’s required by federal statute. As a result, for fifteen years America’s families have been regularly ingesting a group of novel products that the FDA’s own scientific staff had previously determined to be unduly hazardous to human health.

By the time this gripping story comes to a close, it will be clear that the degradation of science it documents has not only been unsavory but unprecedented―and that in no other instance have so many scientists so seriously subverted the standards they were trained to uphold, misled so many people, and imposed such magnitude of risk on both human health and the health of the environment.

—Donald M. Berwick, MD, MPP, FRCP, president emeritus and senior fellow, Institute for Healthcare Improvement; former administrator, Centers for Medicare & Medicaid Services

The Gift of Caring: Saving Our Parents from the Perils of Modern Healthcare Info

    .

The desire to help our elders navigate health issues is clear and universal—how to assure proper care and a good ending is not. Combining adroit storytelling skills with expert advice, The Gift of Caring: Saving Our Parents from the Perils of Modern Healthcare brings the reader into all-too-familiar scenarios facing our aging parents and offers answers to questions we may not know to ask until it’s too late.

Author and biologist Marcy Houle shares her personal journey of caring for her father, a surgeon, who developed Alzheimer’s, and later her mother, who succumbed to other medical conditions. Like many children of aging parents, Marcy often felt powerless traveling this sad trajectory—watching them fall through the cracks of a fragmented and confusing healthcare system, where professionals often wrote off their symptoms as “just old age.” Not having the understanding of the changes that come with aging, she was led to believe there was nothing she could do to help.

The tragic secret? According to coauthor and geriatrics physician Elizabeth Eckstrom, these symptoms frequently are not “just old age.” Rather, the problem is that the current healthcare delivery model for older people is ill-equipped to provide the comprehensive, person-centered care seniors need. Today, thousands of aging people face unnecessary suffering, hospitalizations, nursing home stays, and even death due to complications that could have been prevented or treated. Even more troubling, many healthcare professionals have had little or no training in the care of older adults.

The Gift of Caring reveals these pitfalls and provides families with tools they can use to avoid them. Interspersed with every few chapters of Marcy’s riveting story, Dr. Eckstrom shares professional medical insights, compiled from the latest research, into what Marcy could have done to safeguard her parents. She shows us how to navigate the system, how we can become our loved one’s best advocate, and what we need to know to achieve healthy aging and meaningful, compassionate final years.

Honest, at times humorous, and ultimately uplifting,
The Gift of Caring sheds new light on aging from twin perspectives: a story of a daughter desperately seeking help for the parents she loves, and a geriatrician who gives us the knowledge we need to insist upon a better way.

Based on the authors' teaching and research at the Johns Hopkins Bloomberg School of Public Health and the Keck School of Medicine of the University of Southern California, the second edition of this landmark text offers a general framework for students, researchers, practitioners, and policymakers for learning about vulnerable populations. It contains in-depth data and information on major health and health care disparities by race or ethnicity, socioeconomic status, and health insurance coverage. It is thoroughly updated to include the latest data and trends and provides a detailed synthesis of recent and increasingly expansive programs and initiatives to remedy these disparities. To keep current with recent trends it incorporates the latest Healthy People 2020 objectives, includes new sections on real-world clinical examples, and discusses the impact of health care reform on vulnerable populations. Where Does It Hurt?: An Entrepreneur's Guide to Fixing Health Care Info

    .

A bold new remedy for the sprawling and wasteful health care industry. Where else but the doctor's office do you have to fill out a form on a clipboard? Have you noticed that hospital bills are almost unintelligible, except for the absurdly high dollar amount? Why is it that technology in other industries drive prices down, but in health care it's the reverse? And why, in health care, is the customer so often treated as a mere bystander - and an ignorant one at that? The same American medical establishment that saves lives and performs wondrous miracles is also a .7 trillion industry in deep dysfunction. And now, with the Affordable Care Act (Obamacare), it is called on to extend full benefits to tens of millions of newly insured. You might think that this would leave us with a bleak choice - either to devote more of our national budget to health care or to make do with less of it. But there's another path.

In this provocative book, Jonathan Bush, cofounder and CEO of athenahealth, calls for a revolution in health care to give customers more choices, freedom, power, and information, and at far lower prices.

    You'll learn how:

    • Well-intended government regulations prop up overpriced incumbents and slow the pace of innovation.
    • Focused, profit-driven disrupters are chipping away at the dominance of hospitals by offering routine procedures at lower cost.
    • Scrappy digital start-ups are equipping providers and patients with new apps and technologies to access medical data and take control of care.
    • Making informed choices about the care we receive and pay for will enable a more humane and satisfying health care system to emerge.

Bush's plan calls for Americans not only to demand more from providers but also to accept more responsibility for our health, to weigh risks and make hard choices - in short, to take back control of an industry that is central to our lives and our economy.
